Everyone can recognize a beautiful face. Very few people, however, know exactly what makes it attractive. High cheekbones are an important aesthetic feature in this subconscious assessment. High, beautifully defined cheekbones give a face a charming expression with a touch of sex appeal.
A zygomatic onlay can be used to restore cheeks that have sagged or become sunken over time. For faces with a flat midface structure and a somewhat elongated shape, augmenting the zygomatic bone can result in a marked increase in attractiveness.
The cheekbones are accentuated using grafting materials such as Lyoknorpel or other biological materials. The size and position of the zygomatic onlay are determined on a case-by-case basis with the patient. In this process, every region of the zygomatic complex is carefully assessed: the anterior maxillary/zygomatic region in the area of the infraorbital sulcus, the zygomatic body itself, and the zygomatic arch. This differentiation is important and is often overlooked.
Since the procedure is performed through the mouth, there are no visible scars. The procedure is performed on an outpatient basis under anesthesia and is painless. There is usually only minor swelling after the procedure. The patient can go home afterward. The results are permanent.
Rhinoplasty is a reconstructive surgical procedure designed to improve the appearance of the nose, which may need to be altered due to natural growth, an accident, or a tumor. Nose surgery may also be performed to improve breathing.
More than with any other form of facial plastic surgery, rhinoplasty requires a technically skilled and highly dexterous surgeon with many years of experience who is able to visualize the outcome in advance.
Professor Sailer, who has gained worldwide renown in the field of complex facial deformity surgery, has already successfully performed several thousand nose surgeries. The great art of rhinoplasty lies in achieving what the patient wants, not what the surgeon wants. Many surgeons try to create the same “nose” for all patients. A good surgeon, however, listens to his patients and tries to give them what they want, as long as this can be done responsibly.
Professor Sailer takes both a surgical and an artistic approach to his rhinoplasty procedures. During the initial consultation, he addresses questions, discusses expectations, and outlines possible outcomes. He then performs a thorough clinical examination of the nose to work with the patient to determine which changes are desired and feasible.
To ensure comprehensive surgical planning, the nose is photographed from various angles. In addition, a cephalogram—a true-to-scale lateral panoramic X-ray—is always taken. This is the only way to evaluate the results both before and after surgery. The lateral X-ray is the most valuable tool for surgical planning, as it provides a true-to-scale representation of the nasal and facial structures.
In cases of obstructed nasal breathing and a deviated nasal septum, a CT scan of the nose and/or paranasal sinuses should always be performed. Rhinomanometry (a test to measure airflow resistance through the nose) is also often necessary.
Nose surgery requires a holistic approach. The shape and size of the nose also depend on the other proportions of the face—and indeed, of the entire body. A very tall person should not have a nose that is too small, and vice versa. The nose, lips, and chin form an aesthetic unity. If the chin or lower jaw is small, the nose can easily appear too large. Rhinoplasty always requires precise, comprehensive facial planning. Depending on the degree of difficulty, the procedure is performed either using a minimally invasive technique or through an open approach in the area of the nasal bridge.
A face that is perceived as beautiful always has a well-proportioned and well-defined shape. This includes, above all, the contour of the lower, lateral edge of the face where it meets the neck—the chin and neck contour. Through jaw angle augmentation using an implant, this area can be accentuated, the skin tightened, and a double chin and sagging cheeks permanently reduced.
A person’s natural jaw angle is located at the posterior junction of the lower jaw and the neck. If this contour is poorly defined, the face appears round. Over time, the cheek and neck areas “blend together” due to the sagging of fat and skin tissue. By augmenting the jawline with an implant, this area can be surgically accentuated and the skin tightened again.
This method gives the face a well-defined, shapely contour. The “sagging cheeks” caused by the aging process disappear. The extent to which the implant extends forward along the lower jawline is determined on an individual basis. The implant used is made of a specially developed, biocompatible material whose shape can be customized for each patient.
The result looks natural, leaves no external scars, and is permanent.
The procedure is painless and leaves no visible scars. The treatment is performed under general anesthesia. The patient can go home the very next day. The surgery is performed through the mouth, which is why there are no visible external scars. Swelling is to be expected for about 10 days after the surgery. The position of the teeth remains unchanged. The result is permanent and creates a beautiful contour along the lateral edge of the lower jaw and the neck area. For patients with a small, receding chin, we recommend a combination of mandibular angle augmentation and chin augmentation.
The material is made of Medpor. Upon request, it can also be made from a custom-made PEEK implant. These materials are biocompatible, well-tolerated, and have been used in medicine for decades as replacement materials.
This method permanently removes a double chin and creates a beautifully contoured chin and neckline.
When combined with a chin rim advancement, this procedure replaces the traditional facelift without restricting facial expressions and ensures a lifelong, rejuvenating, aesthetic, and natural appearance.
The chin plays a crucial role in facial aesthetics and contributes significantly to one’s sense of identity. The shape of the chin has an impact on one’s social environment that should not be underestimated. A receding chin, a double chin, and other chin deformities can lead to a negative perception and reduced likability in the eyes of others.
A double chin, sagging chin, or receding chin can be treated with outpatient surgery. Patients often notice a positive change after just a short time: Increased self-esteem is often accompanied by generally better posture, a more confident demeanor, and a more positive aura.
During double chin correction, the chin muscle is tightened again. In the case of a sagging chin, the soft tissues of the chin hang down. Causes include sagging of the musculus mentalis (chin muscle), loss of the anterior mandibular crest, or detachment of the musculus mentalis due to the insertion of an artificial chin implant. A drooping chin can be corrected by tightening the chin muscles. Particularly in cases of a receding chin, highly attractive results can be achieved in the facial profile without the use of an artificial chin implant (see possible complications as described above), thereby enhancing the facial features. The result is permanent.
The procedure is performed on an outpatient basis under anesthesia and is painless. The patient can go home afterward. The procedure is performed inside the mouth, in the lower anterior labial fold behind the lower lip, and leaves no visible scarring. Artificial chin implants are not recommended, as they cause bone resorption in the chin.

Over the course of a person’s life, the eye begins to appear smaller because the eyebrow presses down on the upper eyelid. Bags under the eyes form due to the increased displacement of fatty tissue in the lower eyelid area. The face appears tired and worn out. Professor Sailer has developed new methods to give the entire eye area a naturally radiant appearance.
The traditional facelift method—upper eyelid surgery, in which excess skin and fatty tissue are removed—has the significant drawback of making the eye appear smaller. This method further reduces the distance between the eyebrow and the upper eyelid. The result is often a look of deep-set, hollow, and shadowed eyes.
Professor Sailer therefore developed a new method to give the entire eye area a naturally radiant appearance. To do this, a small incision is made just behind the hairline on the forehead, which leaves no visible scar later on. The drooping eyebrow is lifted slightly, which also eliminates the “crow’s feet” at the sides. At the same time, any remaining excess skin on the upper eyelid can be removed with as little fat reduction as possible; if too much fat is removed, the eye appears smaller. The eye “opens” up again, appearing larger and more friendly. The entire eye area then appears rejuvenated and naturally harmonious.
In addition to older people with sagging skin, young patients often opt for lower eyelid correction or treatment of under-eye bags. Lower eyelid correction is performed through a small, precise incision in the inner mucosal area of the lower eyelid crease—which is not visible from the outside (transconjunctival approach)—allowing direct access to the bags under the eyes. This routine procedure poses no risk to the eye and leaves no visible scars. The conventional method involving external incisions can lead to everted eyelids (ectropion).
If there is a significant amount of excess skin in the eyelid area, an upper eyelid lift can be performed at the same time. This correction is a simple procedure that can also be performed under local anesthesia. In the upper eyelid, the incision is made within the eyelid crease, so that the fresh scar cannot be seen when the eye is open and is virtually invisible once healed.
A thorough analysis is extremely important before any surgical procedure. Eyelid surgery is not the right choice for all patients seeking a more refreshed appearance around the eyes. Often, a brow lift is sufficient to achieve a naturally younger look.
Ever since I was 16, I had been very unhappy with the asymmetry of my face—more specifically, with the unevenness of my cheeks and upper cheekbones—and searched online for a specialist.
Professor Sailer in Zurich seemed to me to be the most experienced in this area, judging by the many photos on his website. When I sent my first written inquiry, I immediately got the impression that they were attentive and responsive to my aesthetic preferences.

I had my surgery with Professor Sailer in October 2017. I had first heard about the clinic several years ago, but the procedure wasn’t feasible for me at that time.

My story begins with the fact that I’ve felt unattractive for years, ever since I was a teenager. My story began when I was 13 years old and the malformation of my facial bone structure set in. My chin and jaw became longer and narrower, lacking volume and masculine features. I thought that gaining weight would be the solution. As I grew up, I realized that the core problem was my bone structure and that it had nothing to do with body fat.
Hoping to find answers and solutions, I began researching and studying the subject. A whole new world opened up to me when it came to jaw and chin correction as well as cosmetic facial surgery, but at the same time, it became clear that the field of facial bone structure is far more obscure than the fields of breast augmentation surgery or rhinoplasty, for which information is much easier to find. “Which surgeon can handle my needs and wishes?”
In Europe, I found a clinic and a doctor who, in addition to other cosmetic surgeries, specializes in facial implants. I decided to have the surgery there. The doctor used silicone implants that look terrible on my face. The result was an asymmetrical appearance, because the silicone implants shifted, and their size, shape, and placement didn’t match my expectations—so my chin became even longer with the implant, and the overall look was completely unbalanced. I was very disappointed. It occurred to me that my case might be too complicated.

After a long and difficult journey, several cosmetic procedures that never left me satisfied, constantly hiding whenever someone wanted to take a side-view photo of me, and various obstacles that had been placed in my path throughout my life due to my malocclusion, I decided in mid-December 2015 to schedule a consultation with Prof. Dr. Sailer.
I always knew that something about my face wasn’t right—or hadn’t developed normally—but not even a doctor could tell me exactly what it was during various previous consultations. Supposedly it was my nose, then my cheeks, then my teeth. No one had a solution or a suggestion on how to go about helping me.
While searching for information on jaw misalignment, I stumbled upon Prof. Dr. Sailer’s website by chance. I read the many positive reviews, and the photos of patients featured there piqued my curiosity about the clinic, the doctor in question, and his team. I sent a non-binding request for a consultation via the contact form, and a day later, Dr. Sailer called me personally to schedule an appointment. So, shortly thereafter, I set off from Germany to Zurich, carrying a fair amount of nervousness but also anticipation in my carry-on luggage. After all, this issue has been on my mind my whole life. Whether back in school or during my college years, I’ve simply always felt uncomfortable.
My first impression of the clinic was great! It’s a warm, cozy practice with a very stylish interior. Everyone on the team was very welcoming and attentive from the very first moment. They explained the surgical options to me in detail. After just this first one-hour appointment, I was already certain that I wanted Prof. Dr. Sailer to perform my surgery. Nevertheless, according to the professor and Dr. Sailer, I should take some time to think it all through carefully—after all, this isn’t a minor procedure, and the financial burden is no small matter either.
After another consultation, during which my questions were answered and Dr. Sailer showed me photos of former patients with similar misalignments, the surgery date was quickly set. I was simply determined and didn’t want to wait any longer, because I was certain this surgery would make a huge difference for me. This was exactly the kind of doctor I’d been looking for over the past few years—one who knows what needs to be done, understands where the problem lies, comes across as trustworthy, and isn’t just trying to sell me something.
So I had surgery about 3 1/2 months after our first meeting. I arrived at the hospital in the morning on the day of the surgery and was incredibly nervous. The professor welcomed me personally, took me to my room, and immediately put my mind at ease. Since Dr. Sailer is generally a very kind and warm person, his demeanor is naturally contagious, and you feel at ease. I trusted him completely at all times. You can tell right away that he loves his job and is fully committed to it. So I got changed, walked into the operating room on my own—or rather, into the room where I received the anesthesia, which, by the way, I tolerated very well—and, to be honest, that’s about all I can remember. I drifted off to sleep, and when I woke up, there was always someone with me; they provided me with drinks and medication. I wasn’t in any pain at that point, and I slept soundly through the night as well.
The next day, however, the swelling set in. But even that was manageable, and contrary to my expectations, I didn’t end up looking like a pumpkin. Of course, the scar in my mouth and the swelling were uncomfortable—it twinged and tugged from time to time—but it was absolutely bearable. Plus, I was given pain medication and everything else I asked for. Since I was feeling so well—I was already walking around the hospital on the second day, chatting with the nurses, taking a shower, and even stepping outside briefly—there was no reason not to leave the hospital after the third night.
That morning, I had one last conversation with the professor—who, by the way, had visited me at least once every day during my stay—and so my mom and I set off from Zurich to Innsbruck, to her home. I figured it would be better to get through the initial swelling from a safe distance—after all, there’s no need to worry anyone at home unnecessarily.
Unfortunately, however, my condition got progressively worse in Austria. I was in severe pain, had recurring nosebleeds at night, circulation problems, and difficulty breathing because my nose was completely blocked. My entire jaw just hurt incredibly.
Dr. Sailer had given me his personal number earlier, so I called him on the second day after I was discharged and told him about my pain. Just hearing his voice put my mind at ease. He explained that it couldn’t be anything serious, told me what to do, and immediately sent a prescription for stronger pain medication to the pharmacy, which my mom was able to pick up just 15 minutes after the call. Fortunately, the medication worked quickly, though I have to say that the next two days were still very painful and exhausting. Dr. Sailer also called me the next day and personally checked in on how I was doing. He really went out of his way to help. After a total of about four painful days, things started to improve day by day. The swelling went down more and more each day, and the contours became increasingly visible.
Right now, exactly four weeks after the surgery, I could cry every time I look in the mirror. I’m so happy! I’m so glad I decided to take this step, and I can feel my self-confidence growing. This surgery really isn’t a minor procedure—as I mentioned earlier—and you should be aware that you’ll be limited for the first few weeks: you won’t be able to chew, and in the worst-case scenario, half your face will be numb, which makes even drinking—or bringing a glass to your lips—difficult. But it’s worth it. And that’s exactly what I thought about every day: It’ll be worth it. For this result, the pain and temporary limitations were simply worth it.
Prof. Dr. Sailer’s work is simply incredible; even today, I still can’t believe what he’s done for my face. On top of that, he and the entire team are so kind and approachable. Dr. Sailer has also gone above and beyond—she held my hand while the stitches were being removed, assured me I could call at any time, cheered me up when I was experiencing pain, and answered every question in detail both before and after the surgery. I felt well taken care of the entire time, whether at the clinic or at home. I’m looking forward to the coming weeks and to the final result, even though it already looks great today. The most important thing for me: I feel beautiful and truly comfortable in my own skin—for the first time in my life!
